Job Description
The Family Mentor provides one-on-one support to clients by offering guidance and support to people who are struggling with substance abuse. The Family Mentor works closely with a variety of professionals to connect parents to treatment resources and provides for the safety and well-being of the children and families by delivering culturally competent services to the unique needs of the family.
Essential Functions
- Provides for the safety, stability and well-being of children and families during the referral period
- Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models
- Meets DCF timelines to connect with the family and facilitate the initial family meeting
- Engages and works in partnership with the case manager and family to complete assessments
- Responds promptly to family crisis situations
- Meets with clients at least once a week to offer one-on-one support
- Develops recovery plans, coping methods and rehabilitation services with the client
- Facilitates treatment assessments and referrals and provides client support in arranging for medical appointments, counseling, support groups, and other resources as needed
- Monitors the client's behavior, schedule, medication, and activities and reports any unusual behavior, conflicts, or recovery deviations to the relevant professionals
- Provides transportation and resources and coordinates community support services as needed
- Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
