Job Description
Under the supervision of the Area Director, the Case Manager shall be responsible for case management services provided to the clients served. The Case Manager shall operate in an ethical, professional manner, and interact with others in such a fashion as to create and maintain respect for the agency. They shall act in accordance with the policies determined by the Board of Directors and shall abide by all applicable federal, state and local regulations. Specifically, those related to Social and Rehabilitation Services/ Commission on Substance Abuse, Mental Health and Developmental
Disabilities/Substance Abuse Treatment and Recovery Licensure Standards and any contractual agreements. The duties of this position include referral services, educational presentations, individual counseling, group counseling, case management activities and all related paperwork demands.
1. The case manager facilitates educational presentations as needed and required.
2. Provides case management and referral services for program participants in the following basis areas of human need as required: employment, vocational training, continued education or GED testing, community resources, legal assistance, housing, family enrichment, family violence, and other client needs.
3. Meets with clients at least once a week to follow up with life plans and goals.
4. Tracks and collects pertinent data for grants; and reports and completes all required reports and summaries in a timely manner for the Area Director.
5. Works within the program team to deliver programs which align with outcome objectives set by the team.
6. Provide a written case management plan for all the off enders assigned to your caseload at the Mirror.
7. Participates in recruitment, orientation, training, and smooth transition into coaching for all clients.
8. Must attend and participate in all staff meetings.
9. Provides individual sessions with program participants and assist them in the identification and resolution of problems, behaviors or cognition, which prevents them from completing the training, and finding and retaining employment or fostering family effectiveness.
10. Maintain paperwork, and reporting outcomes in a timely manor.
11. Facilitate and make appropriate program participant referrals to community-based services related to their personal needs consistent with Mirror policy and guidance from the grantor.
12. Perform other job responsibilities as assigned by the Area Director or members of the Mirror Management Team.
