Transitional Housing Case Manager

Job Description

The Transitional Housing Case Manager assists in supporting clients to develop goals toward education, employment, life skills, and other social skills. The Case Manager will develop client skills, enabling them to obtain permanent housing, while engaging in substance use disorder services to live a life in recovery.

Essential Functions

  • Provides for the safety, stability and well-being of clients and families during their stay in DCCCA's Transitional Housing Program
  • Delivers culturally competent, strengths-based case management services to clients consistent with the evidence-based practice models
  • Collaborates with community partners to assist them in completing day-to-day case management
  • Completes DLA-20 screening on all clients upon admission, and every 60 days thereafter
  • Engages with clients to complete assessments and develop case plan goals and objectives
  • Attends weekly staff meetings at DCCCA's residential program in Lawrence, KS
  • Attends weekly staff meetings with DCCCA Family Preservation staff
  • Completes sex offender registry search on all tenant applicants
  • Completes weekly checks of home to ensure cleanliness and safety
  • Provides Safe Sleep Instruction for all women with children under the age of 1 year
  • Assists with referrals for additional support services
  • Coordinates admission process for DCCCA outpatient substance use disorder groups
  • Assists clients in developing and following a budget
  • Meets with clients weekly to discuss progress, barriers, and identify any additional support needed
  • Completes a medication inventory and reconciliation on all clients and families upon admission
  • Assists clients in completing a housing voucher application on the day of admission, if not yet completed
  • Completes program agreement with all clients on the day of admission
  • Completes Transitional Housing admission paperwork prior to move in date
  • Regularly communicates with Tenants to Homeowners to schedule client move in dates, move out dates, and maintenance needed on homes
  • Completes vehicle searches on client vehicles when necessary
  • Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the client and their family
  • Collects drug screens as necessary
  • Documents all client interactions in the Electronic Health Record
  • Maintains confidentiality for all clients and their families by adhering to HIPAA and CFR 42 Part 2 Guidelines
  • Promotes and practices teamwork in all activities
  • Fosters and maintains professional relationships with families, community partners, and peers
  • Participates in professional development that is relevant
  • Completes Housing Treatment Plan within 7 days of admission to program
  • Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required
