What you'll Do
The RN or LPN is a key member of the care team within a Certified Community Behavioral Health Clinic (CCBHC). In this role, the nurse supports the Family Nurse Practitioner (FNP) and collaborates with a multidisciplinary team to deliver integrated, patient-centered care. Responsibilities include providing clinical support, assisting with care coordination, and helping ensure that patients’ physical and behavioral health needs are addressed in a comprehensive and compassionate manner, consistent with scope of practice.
Essential Responsibilities
Direct Patient Care Support:
- Support licensed providers (APRN, PA, MD, DO) in delivering safe, high-quality care to patients across the lifespan, consistent with scope of practice.
- Obtain and document vital signs, health histories, and other relevant clinical information; contribute to patient assessment through data collection and observation.
- Prepare patients and clinical environments for examinations and procedures; assist providers during visits and interventions.
- Administer medications and treatments as ordered by a licensed provider—including injections, immunizations, and basic wound care—in accordance with licensure and organizational policy.
- Perform routine clinical tasks such as specimen collection, point-of-care testing, EKGs, and other diagnostics as ordered.
- Provide patient and family education related to treatment plans, medications, and general health and wellness, reinforcing provider guidance.
- Observe, document, and report patient responses to care, identifying and communicating changes or concerns to the provider and care team in a timely manner.
Care Coordination & Whole-Person Support:
- Collaborate with a multidisciplinary team to support integrated, whole-person care, addressing physical health, behavioral health, and social needs.
- Assist with care coordination activities, including referrals, follow-up appointments, and communication across internal and external providers and services.
- Support screening and monitoring activities (e.g., health status, functional needs, or risk factors) using standardized tools, as directed.
- Help patients and families navigate healthcare and community systems by providing information on available services and resources.
- Reinforce continuity of care by tracking follow-up needs, supporting outreach efforts, and ensuring relevant information is communicated across the care team.
- Promote a patient-centered, trauma-informed, and culturally responsive approach in all interactions.
