Job Description

The Healthy Babies Community Liaison Coordinator provides for the day-to-day supervision of the Healthy Babies Community Liaison staff. Performs administrative work to assist the Healthy Babies Program Manager. Participates in the planning and direction of the Healthy Babies Program, including personnel management and team meetings. Reviews complex and/or difficult office and field procedures and the development of procedural improvements. This position works to meet the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program’s focus on reduction of infant mortality, low birth weight, and prematurity among high-risk moms. Employee will provide support during public health emergencies (Activation Level 1).

 

Coordinate daily home visitor activities.

  • Manage and evaluate personnel outcomes and expectations as they are related to grant requirements.
  • Review weekly and monthly reports regarding home visitor compliance as they relate to grant outcomes.
  • Work with the Program Manager and Senior Admin. Officer to ensure program outcomes are met.
  • Meet with home visitors on a consistent monthly basis to discuss progress on meeting program outcomes.

 

Compile monthly data reports to track timely and accurate completion and submission of program-required documents.

  • Monitor and manage home visitors’ caseloads to ensure equal distribution of client cases and maintaining expected caseload.
  • Monitor weekly and monthly reports and provides feedback as requested to the Healthy Babies Program Manager.
  • Monitor home visitors’ progress towards achieving program requirements as stated in program grants.
  • Use tools such as data tracking spreadsheets to track home visitor progress.
  • Establish clear deadlines for home visitors to complete tasks as assigned.
  • Review electronic charts as part of their monthly supervision with home visitors to ensure accuracy, completeness, and that program outcomes are being met.
  • Ensure program quality by completing quality assurance audit checks on client medical records and home visitor documentation.

 

Participate in and facilitate building/maintaining partnerships with both internal and external partners.

  • Represent Healthy Babies in a positive manner and promote the program goals and mission to foster positive support from community partners.
  • Attend and participate in community coalitions as requested.
  • Provide Healthy Babies program objectives and participant criteria to community partners as requested.
  • Promote and foster provider participation by participating in at least 3 community outreach events in addition to the Community Baby Showers hosted by Healthy Babies.
  • Lead and/or assist with quality improvement processes within the agency.
