Job Description
GENERAL DESCRIPTION OF POSITION
The Health Promotion Department works to improve health outcomes, reduce the overall cost of care, and improve the patient experience within identified patient populations. These populations are identified through advanced healthcare analytics, quality metrics, and provider and care team referrals.
Under the direction of the Health Promotion Manager, this position supervises, organizes, and monitors the day-to-day activities and work of the Health Promotion Nurses.
RequirementsESSENTIAL DUTIES
- Oversees the day-to-day activities of the Health Promotion Nurses, including assigning outreach tasks/responsibilities, hospital admissions/emergency room follow-up activities, and Accountable Care Organization (ACO) activities.
- With the assistance of the Health Promotion Manager, consistently assesses the organization and structure of the workload of the Health Promotion Nurses, including updating workflows, updating distribution of tasks, and updating goals/expectations for Health Promotion Nurses.
- Makes sure that all Health Promotion Nurses are appropriately cross-trained on transition of care functions and chronic care management functions.
- Makes sure that all Health Promotion Nurses are knowledgeable about the resources within CHC/SEK and resources (both healthcare and social services) within the communities CHC/SEK serves.
- Monitors, reviews, and assesses notifications and information available to Health Promotion Nurses; including maintaining knowledge/understanding of notifications and information provided in electronic health record (EHR) reports and payer portals while also?collaborating with outside facilities to obtain timely discharge records and needed information.
- Utilizes technology and telehealth platforms to better help Health Promotion Nurses connect patients to care and provide nursing assessments, as needed.
- With the assistance of the Health Promotion Manager, contributes to and utilizes data (e.g. ACO information) to achieve program goals and improve patient health outcomes.
- Assist in educating providers and support staff in value-based care initiatives, particularly around hospital visits/discharges and chronic care management.
- Participate in quality improvement activities to continually help CHC/SEK improve care gaps and improve patient outcomes, including identifying opportunities to implement best practice approaches and innovations to improve outcomes.
- Actively participate in ongoing in-service programs, staff meetings, continuous quality improvement, and periodic performance/protocol evaluations and development.
- Understands, is knowledgeable, and can otherwise perform all the essential duties of a Health Promotion Nurse.
- Execute all duties and services in full compliance with CHC/SEK policies and procedures.
