Job Description

Job Summary: Under the Supervision of the Regional Area Director or their designee, the

Community Based Care Coordinator shall be responsible for person centered,

strengths-based case management services for individuals seeking treatment

services. The Community Based Care Coordinator shall operate in an ethical,

professional manner, interact with others in such a fashion as to create and

maintain respect for the agency, its partners, and its stakeholders. The Community

Based Care Coordinator will be part of a strong multidisciplinary team, working to

serve our clients and communities. The Community Based Care Coordinator will

assist clients with developing person-centered goals (finding jobs, housing,

developing hobbies, building supports) while maintaining a healthy lifestyle.

QUALIFICATIONS:

1. Must be certified by the Kansas Department of Aging and Disability Services as a Person-Centered

Case Manager prior to beginning employment in this position.

2. High school graduate or equivalent.

3. Ability to navigate EHR

4. Minimum of one-year paid experience providing case management or workforce development

services to a similar population.

5. Although not mandatory persons that are bilingual (English & Spanish) will be given strong

consideration.

6. Excellent interpersonal skills required as well as ability to communicate effectively both verbally

and in writing.

7. Capable of maintaining a team spirit and the ability to work cooperatively with others.

8. Must be adaptable to change and have effective organizational skills.

9. Must be able to organize and follow through on multiple tasks.

10. Display consistent, good judgment and decision-making skills.

11. Ability to establish and maintain rapport with target population.

12. Must be able to pass background/security check.

13. Employees who have been diagnosed with any type of substance use disorder or behavioral

addiction/compulsion must have sustained a minimum of two consecutive years of abstinence from

all intoxicating substances and symptoms of their chemical and/or behavioral addiction; and must

be in full sustained remission.

14. Must be proficient in Microsoft Office Word. Outlook and with a basic understanding of Excel

preferred.

15. Position may also involve bending, walking stairs, and lifting up to-40 pounds on occasion.

16. Position will work in a variety of environments including office settings, correctional facilities, and

outdoors.
