This position is a part-time, benefits eligible position at 20 hours per week.
Job Description:
Job Duties:
Case Management - 70%
- Follow up on referrals and enroll new participants, completing all required tracking and documentation
- Identify barriers to program enrollment and participation
- Promote evidence-based health promotion and risk reduction strategies related to pregnancy, health and safety
- Meet with participants in person and work to identify needs through home visitation, screening and conversations.
- Administer and score screening tools and provide brief intervention related to relevant topics
- Coordinate connections to other programs to meet the needs of the participants, including:
- Refer through established partnerships with area programs and track outcomes of these referrals
- Ensure follow-through with enrollment and participation as needed
- Provide assistance in navigating through various systems related to access to services
- Identify barriers and assist clients in strategizing to overcome these obstacles (e.g. transportation, childcare)
- Report issues outside of scope to Program Director
Program Planning, Coordination, Implementation - 15%
- Develop, monitor and maintain referral processes for the LYFTE Program
- Provide regular status updates to internal personnel and external partners regarding successes, barriers and program needs
- Work collaboratively with other program staff and community partners to resolve conflicts and overcome barriers
- Participate in decision making processes related to quality improvement
- Facilitate biweekly program updates with the Program Director
- Distribute promotional program materials within the community
- Represent LYFTE with referral agencies, stakeholders and at community events
- Participate in collaborative work with other state-wide sites, including attendance at relevant conferences, trainings and workshops
- Support special projects to improve maternal and infant health, including promoting research opportunities to program participants
Data Collection and Evaluation - 10%
- Obtain necessary authorizations and consent from clients
- Enter data relative to the program in a timely and accurate manner
- Review program data (with the executive team) to utilize in program planning and continuous program improvement
- Use referral monitoring processes to inform on improvement and effectiveness
Administrative and Grant Support - 5%
- Attend all funder required trainings, webinars and conferences
- Assist with the completion of quarterly grant reports
- Assist in the development of annual grant applications
- Participate in collaborative work with other state-wide sites, including attendance at relevant conferences, training and workshops

Work Environment:
- This position is in-person with no remote work option.
- Office is location on the Wesley Medical Center campus in the Wesley Medical Arts Tower.
Required Qualifications:
Education: High school diploma or GED.
Work Experience:
- 2 years' experience working in one of the following areas: with community-based programs, heath care systems, health departments, or clinics
- Experience with databases, email, the internet, Zoom, Teams, Microsoft Word, Excel, and PowerPoint
Preferred Qualifications:
Education: Bachelor's degree in health, social work, psychology or related field
Work Experience:
- Bilingual in Spanish or other relevant language
- Case management training or experience
- Experience working with adolescents and/or pregnant and postpartum women
- Knowledge of maternal infant health and evidence-based safe infant care practices
- Knowledge of perinatal needs, education and employment programs, and related services in Wichita
Skills:
- Excellent interpersonal skills and the ability to work effectively with others, in person, by phone and by virtual platform (e.g., Zoom, Microsoft Teams)
- Self-directed and able to exercise independent judgment
- Effectively organize and concurrently manage daily tasks
- Maintain confidentiality
- Experience with data collection and entry
- Critical thinking skills
Comprehensive Benefits Package:

Compensation Statement:
The pay range listed for this position is determined by our compensation program using market data and salary benchmarking. A combination of factors is considered in making compensation decisions including, but not limited to, education, experience and training, qualifications relative to the requirements of the position, and funding. At the University of Kansas Medical Center, a reasonable estimate for the starting pay range will be the minimum to midpoint of the posted range, taking into account the combination of factors listed above.
