Bachelor’’s Degree and a minimum of at least six months experience in the field of services to individuals who are developmentally disabled or relevant experience in the field, which may substitute for any portion of a Bachelor’’s Degree at a rate of six months of experience for each semester of college.
Completes assessments such as individual and family interviews, arranges for professional evaluations, compiles and/or completes referral or placement packets. Provides service planning and coordination of service functions. Supports community inclusion and advocacy in service delivery. Communicates with all support and /or service providers. Develops and implements the Person Centered Support Plan (PCSP). Maintains records on clients in quarter-hour increments of time spent.
