As part of the primary care treatment team, the behavioral health consultant identifies triages and manages patients with physical and behavioral health problems within the primary care setting with a strong focus on substance use and Medication Assisted Treatment referral. In addition, the behavioral health consultant provides skill training through psychoeducation and patient education strategies and develops specific behavioral change plans with patients through use of motivational interviewing and patient activation techniques. This innovative program is a partnership with GraceMed and focuses on improving physical and behavioral health outcomes of clinic patients and a focus on expanding access to substance use disorder services and Medication Assisted Treatment.

Screening, Referral and Consultation:
•Brief screening using PHQ-9, GAD-7, SBIRT, assessment and diagnosis
•Delivers psychoeducation for patient and their support systems
•Medication adherence counseling and disease self-management counseling.
•Motivational interviewing to develop behavioral strategies aimed at symptom reduction
•Provide target oriented cognitive behavioral interventions aimed at modifying negative thinking and promoting self-efficacy
•Self-Care plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction and relaxation
•Substance Use screening, identification of maladaptive coping strategies, development of harm reduction strategies and referral and,
•Consultation with primary care health professionals to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans

Documentation and Care Coordination:
•Provide consultation to and coordinate care of patients with clinic primary care staff
•Identify, refer and advocate for patients needing specialty behavioral health services and other services as needed
•Be immediately available to the clinic primary care staff during working hours in the event of a behavioral health or substance use emergency
•Evaluate crisis situations and apply appropriate interventions
•It is expected that the behavioral health consult utilize concurrent documentation strategies

Administrative Functions:
•Actively participate in meetings that support COMCARE and GraceMed’s integrated health care model to provide comprehensive care for patients
•Assist in developing care management processes and care guidelines to improve self-management of chronic disease
•Assist in provider training and education in integrated behavioral health-primary care model
•Participate in quality improvement activities
•Work from an interdisciplinary approach in a seamless fashion

Minimum Qualifications: Master’s degree in social work, with a minimum of 2 years of experience as a therapist and a current license (or eligible for licensure) to practice in Kansas as a Licensed Clinical Master Level Social Worker. Minimum of 2 years’ experience. Must have a valid Kansas Driver’s license and current proof of automobile insurance.

Preferred Qualifications: LSCSW with a minimum of 3 years’ experience as a therapist and a current license (or eligible for licensure) to practice in Kansas as a Licensed Clinical Social Worker Dual licensure as a Licensed Substance Abuse Counselor. Five or more years delivering brief interventions.
