Social Worker – Other Patient Support Services – Full-Time, Rotating – Via Christi Health – Wichita, KS
Full – Time Monday – Friday Varied Hours ( 8:00 a.m. – 5:00 p.m. and 10:00 a.m. – 7:00 p.m.)
This position works with a centralized health services team and is non-participant facing 90% of the work week. This position will train for and facilitate Individual and group telephonic/virtual health classes and consultations to support Behavioral Health self-care management.

What You Will Do
As an Associate with Via Christi Health, you will have the opportunity to investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
+ Interviews patients, assesses priorities and documents case activity.
+ Consults with team managers, statutory and voluntary agencies and patient’s relatives to ensure compliance with patient’s treatment plan.
+ Assists clients/families with obtaining community assistance by referral to proper resources.
+ Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
+ Supports management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
What You Will Need
+ Required Credential(s):
+ One or more of the following:
+ Specialist Social Worker specializing in Clinical Practice credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
+ Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) in the state of Kansas by the Kansas Behavioral Sciences Review (KSBSRB) required or must be obtained prior to hire or job transfer date.
+ High school diploma/GED with 2 years of experience, or Associate’s degree, or Bachelor’s degree required.
+ 3 years of experience preferred.
+ Prefer 3 years experience working in Behavioral Health setting supporting the care of patients with, depression, anxiety, stress management needs, or bi-polar
+ Prefer clinical setting experience
+ Prefer experience in Motivational Interview skill-sets
