Job Summary

As a Home Health Registered Nurse Case Manager, you play a crucial role in leading care for clients who choose to receive care in their home. You have excellent clinical skills, work well with families, clients and caregivers, and are able to manage multiple client responsibilities.

What you’ll do: A typical day will include managing and overseeing multiple tasks defined by a client service plan. You’ll provide multiple types of care and contribute to the client’s social, emotional and physical well-being. You’ll provide care as directed by their physician and help educate the client, their family and caregivers about the care you provide. You’ll help maintain self-esteem, and communicate with the client opportunities for their independence in decision making. You’ll adhere to required privacy and confidentiality regulations and document your work.

Graduate of an accredited school of nursing. Prior work experience in like or related position may be helpful. Licensure for state of practice is required.
Knowledge of basic nursing skills.
Basic ability to communicate and comprehend; ability to measure and comprehend certain quantities; knowledge and familiarity with the therapeutic effects and side effects of all medications administered.
